Babies First
At CareSource, we want our members to have happy and healthy babies. That is why we created our Babies First program. In this program, members can earn up to $110 in gift cards to local stores like Meijer or Wal-Mart just for keeping medical appointments. It’s that easy.
The information below shows the coupons you can get:
Coupon A – Have your first prenatal appointment during your first 3 months of pregnancy. This is worth a $10 gift card.
Coupon B – Keep all scheduled prenatal appointments during your second trimester. This is worth a $10 gift card.
Coupon C – Keep all scheduled prenatal appointments during your third trimester. This is worth a $10 gift card.
Coupon D – Mother must be seen by the doctor 3-8 weeks after delivery. This is worth a $10 gift card.
Coupons E-K – Baby must get preventive checkups, called EPSDT (Early and Periodic Screening, Diagnosis and Treatment) exams, at 1 month, 2 months, 4 months, 6 months, 9 months, 12 months, and 15 months of age. These are worth a $10 gift card for each visit.
How to participate
- Call the CareSource service center a 1-800-390-7102 (TTY for the hearing impaired: 1-800-649-3777 or 711) to request Babies First brochures with coupons if you do not have them.
- Follow the instructions on each coupon.
- Mail them to the address indicated.
You will get your gift cards for complete and verified coupons. The brochures and coupons explain all coupon rules including redemption time limits.
