National Provider Identification Number (NPI)

CareSource is committed to being compliant with the NPI Initiative set forth by the Centers for Medicare and Medicaid Services (CMS) and corresponding requirements that have been implemented by Michigan Department of Community Health (MDCH).
MDCH announced that they will be ending the NPI contingency plan for fee-for-service Medicaid effective October 1, 2007. CareSource will be consistent with MDCH beginning January 1, 2008, and will require providers to submit all claims - electronic or paper – with NPI numbers and a tax ID number.
Claims received without these numbers will be rejected. Use of these identifiers will ensure your claims are paid accurately and timely.
All claims submitted to CareSource should include the following identifiers effective January 1, 2008:
- NPI (all relevant NPIs should be included)
- IRS Tax ID number
If you wish to continue submitting your legacy provider ID number, you may, in addition to the required information listed above.

Apply for an NPI:If you do not already have your NPI, submit your application online at no cost through the National Plan and Provider Enumeration System at https://nppes.cms.hhs.gov or by calling 1-800-465-3203 (TTY: 1-800-692-2326).
